Medication Refills
We have a 48 hour (two business days) target turnaround time for all medication refills. Please make note of your current supply and contact the office at least 2 days prior to running out of your medications which does not include weekends.
All controlled substances need to be filled at each scheduled appointment. The patient is responsible for making sure your appointment falls prior to running out of your medication. If a controlled substance refill is needed in between appointments, there may be a charge for this refill.
Controlled substance refills will not be provided to patients who have canceled their follow up appointments. This is the responsibility of the patient to keep appointments. Our office requires an appointment every 3 months or 90 days for controlled substance refills.
A drug screening may be obtained at any time while a patient is prescribed controlled substances. This is currently at the discretion of the provider.
Appointments
We charge $25 for any missed appointment and up to $25 for any appointment canceled or changed within 24 hours of the visit. Our staff cannot reverse these fees. Arriving late for an appointment is the same as missing it. All patients have a 10 minute grace period to arrive for their appointments. If you will be outside of this time frame please contact the office. The appointment may need to be rescheduled as time allows.
If a patient or family member needs to speak to the physician or nurse practitioner an appointment will need to be scheduled. If they are able to give a call back, a tele-health appointment will be scheduled.
Our clinic schedule is not always available for work ins or same day appointments. As time and appointments allow, we will try to work patients in for same day appointments. Please call the office for an appointment. If you are in need of a same day appointment and we are unable to schedule you, please visit urgent care or the emergency room based on the severity of your symptoms.
We do not see new patients for emergencies, urgent sick visits, worker compensation injuries or for treatment after a motor vehicle accident.
Self-pay
We do currently accept self-pay patients. This is subject to change at any time. Please contact our office for more information and pricing.
Our self pay rate has increased for 2024. If you are a self-pay patient, you should receive a self-pay agreement outlining the current pay rate.
Our office does not bill for labs or other diagnostics tests. If you are self-pay patient and have a need for labs or diagnostic testing, you will be responsible for any payments required.
We are willing to discuss weight management with patients, however, this is not often covered by insurance. You are responsible to contact your insurance to determine if they cover weight management services. If they do not cover weight management services, please contact us to discuss our self pay rates.
Partnership
Patients of Partners in Health Family Medicine share in the responsibility for their own health care and must communicate openly and honestly with us.
As a patient, you will be expected to follow recommendations, complete medication regimens, and show up for scheduled exams, referrals, procedures and follow-up appointments.
You have the responsibility to treat us, our staff, and your fellow patients with the same courtesy you feel you deserve. Hostile or abusive behavior will not be tolerated and may result in dismissal from practice.
The office will have a soft closure during lunch between 12PM-1PM and phones may not be attended during this time.
We try to be available for phone calls throughout the day. Due to our current schedule and staffing, phones may not be attended at all times during office hours. Please leave a message if needed. Messages may not be returned on the same business day.
Disability
We are not a disability clinic and functional limits or disability ratings are not performed by this office.
We do not currently participate in disability determinations or worker’s compensation. If your medical records are needed for either of these, please contact our office. A signed medical release may be needed to release medical records.
